"It should be noted that in our study, a gluten free diet
was only given to those patients who were proven to
have coeliac disease.
It is well known that irritable
bowel syndrome may respond to changes in diet.16
Therefore, it may be argued that a major dietary
change, such as the strict gluten free diet given to
coeliac patients, may also favourably affect symptoms in
noncoeliac irritable bowel syndrome patients.
The
placebo effect of this diet is also considerable. The
probable effect of a gluten free diet on irritable bowel
syndrome patients has been studied by Wahnschaffe
et al.12 As part of a larger study, they gave a gluten free
diet to 26 irritable bowel syndrome patients.
Only those
irritable bowel syndrome patients with markers of
coeliac disease, such as HLA-DQ2 and coeliac specific
antibodies in duodenal aspirate, responded. The authors
suggest that these markers may indicate potential or
latent coeliac disease in this subgroup of irritable bowel
syndrome patients.
They did not find any improvement
in symptoms of irritable bowel syndrome patients
without markers of coeliac disease. We believe it is
reasonably safe to attribute
the improvement of symptoms
in our subjects to the specific effect of gluten free
diet on coeliac disease, rather than to placebo effect."
